Electronic Signature
– If the signature is electronic, it must have an IP Address or a computer generated time-stamp.
– If a release has a date of signature that is typed and the signature is also typed, the signature font must be typed in a “signature style” OR have an IP address.
Hand written Signature
-The date of signature must also be hand written. (not typed)
Valid Dated Signature.
-Has to be signed within 90 days of the background check (Unless the consent has specified wording such as “for the duration of my employment”.
– Must be a complete signature (No initials or picture will be accepted.)
Consents/releases CANNOT have:
– strike-throughs, black-outs, white-outs, scribbles, or any other alterations.
